A Grassroots Initiative for End-of-life Planning

We were very pleased to see the article by Maureen Bisognano and Ellen Goodman (Feb 2013). Although we truly appreciate the work of the Institute for Healthcare Improvement and the Conversation Project, we think the authors missed an opportunity to highlight perhaps the largest grassroots advance care planning initiative currently under way in the United States.

Honoring Choices Minnesota®—an initiative of the Twin Cities Medical Society—has been operational since 2008 and is a model that other states and communities are beginning to implement throughout the country. It is an on-the-ground program that does exactly what the Conversation Project is advocating.
