Making the Most of The Conversation Project Resources and Getting Started

    Join Dr. Erik Fromme from Ariadne Labs and Kate DeBartolo from The Conversation Project as we launch our new workbook: What Matters to Me – A Patient’s Guide to Serious Illness Conversations.   This workbook is designed to help people with a serious illness get ready to talk to their health care team about what is most important to them — to make sure that they get the care they want.   • Improve how you reach and change public perceptions of advance care planning September 2024

    Join us with special guests Anthony Back, MD, University of Washington and Marian Grant, DNP, RN, content and marketing consultant, both from The John A. Hartford Foundation- and Cambia Foundation-supported Message Lab project. Together we will explore how organizations are using evidence-based messaging principles to improve how they talk about advance care planning. Learn how to apply strategies to your own promotional materials, tools, website, etc. and share your ideas.

  • Supporting Veterans – Understanding What Matters Most November 2023

    Join us in honor of National Veteran/Military Family month as we hear from special guest Dr. Qwynn A. Galloway-Salazar, Founder, In Their Honor, LLC (and others) discuss ways to engage, support and better understand what matters most to Veterans in your community in order to support care planning conversations.  November is also National Family Caregivers month so bring your insights, ideas and questions to share with each other on engaging and supporting caregivers.

  • Community Planning 101  August 2021

    This 35-minute recorded session provides community leaders with an overview of The Conversation Project, how individuals/groups are bringing this work into their communities and how you might approach and introduce the importance of care planning conversations into your community. This session will help you think about how to reach individuals where they live, work, pray and learn by providing you with guided questions to consider for your region, including goal setting and tips on approach. And, we’ll introduce you to the free resources we and others have to help you take action.

  • Inspiring Conversations Through the Power of Film and Virtual Theater March 2021

    Looking for fresh ways to inspire conversations and prompt action toward Advance Care Planning right now in our virtual world (and always)?  We’ll hear from Dr. Jessica Zitter (Director/Producer of Caregiver: A Love Story and physician whose work is featured in Extremis), Elizabeth Coplan (Playwright of ‘Honoring Choices’ virtual plays), Rev. Corey Kennard (Healthcare Activist and Lead Pastor of Detroit’s Amplify Christian Church), and learn about a new upcoming documentary on aging, Fast Forward. Our guests will highlight examples of how using film & virtual theater can help spark stronger conversations and actions within communities during these difficult times. Join us to learn how you might do something similar in your community and hear about upcoming opportunities you can tap into.

    Join this call to learn and share best practices regarding messaging and developing alternative NHDD activities in the midst of COVID-19. During the call, you’ll hear from two leaders from The Conversation Project, Kate DeBartolo and Patty Webster, as well as from the Executive Director of the Massachusetts Coalition for Serious Illness Care, Anna Gosline. We understand that we are living in uncertain times and that how we continue to move forward with our work requires careful and critical thinking. Our goal is for participants to leave this call with practical steps to successfully navigate this pandemic and execute their meaningful work in a way that aligns with the current times. This call is an opportunity for us to collectively learn from one another and share what practices have been most successful in our communities.

  • Reaching and Supporting Solo Agers November 2019

    Are you grappling with how best to support those who may not have family or a support system around them? Or are you reaching “elder orphans” and have some ideas to share on how best to ensure this population is supported, listened to and given opportunities to talk about what matters most to them?  Then this call is for you! Join The Conversation Project to discuss ways to ensure this critical segment of the population is not left behind. We’ll be joined by a solo ager who created an expansive support network for others and has learned some fabulous lessons along the way.

  • Lessons on Messaging from the Massachusetts Coalition for Serious Illness Care July 2019

    At their May 16th summit, the Massachusetts Coalition for Serious Illness Care shared the results of their messaging research project. During our July 17th community call, they shared these latest findings and presented learning from their summit, including tested language for engaging the broader public, as well as targeted “segments” of the population, in conversations about what really matters to their life and health.

    Aimed at giving more specific guidance to leaders who have already established The Conversation Project in their region, this webinar discusses tactics to keep your work fresh in order to sustain momentum, as well as strategies to reach new or hard-to-access audiences. Learn from the experiences of Frederick Memorial Hospital in Maryland, whose ACP team runs a comprehensive, multi-stakeholder community campaign to increase conversations and completion of Advance Directives via community education and programming about advanced care planning.

    This 45-minute recorded session provides community leaders with the knowledge and resources needed to host a Conversation Starter Workshop event. Training includes how to give an informative presentation, weave in latest messaging, and facilitate interactive group discussion. You’ll be given an example of a standard presentation and tips on all phases of event-planning including organization, facilitation, and evaluating your event.
