Helping people share their wishes for care through the end of life.

My Health Checklist

We are happy to share this new Age-Friendly Health Systems resource for older adults. This free guide is designed to help you get the most out of your medical appointments. It helps you think through all aspects of your health — what’s going well, what could be better, and your questions or concerns. Being prepared means that you can make sure your appointment focuses on the things that matter most to you. Coming soon, we’ll also have translations of the guide in Chinese, Portuguese, and Spanish.
