• All of our kits may be downloaded and printed from our Starter Kit page, or copies can be ordered directly via our online marketplace.

Co-branding

For organizations planning to distribute any of our kits, we have a version to which you can add your logo and local contact information. All of our kits may be co-branded one of two ways:

1. Order through our online marketplace — order as many or as few copies as you like
   • Co-branded Conversation Starter Kit
   • Co-branded Conversation Starter Kit for Loved Ones of People with Alzheimer’s/Dementia
   • Co-branded Health Care Proxy Kit
   • Co-branded How to Talk to Your Doctor Kit
   • Co-branded Pediatric Starter Kit
2. Request a PDF copy which you can print yourself (see an example HERE)
   • $250 per for two kits/languages; $50 for each subsequent kit
   • We can also send you a link to a digital version of the kit to distribute

• A list of requested kits
• Organization name (for front page)
• Contact information (for back page)
• Logo (for back page)
• Mailing address for invoice
• Preliminary estimation for how many copies you intend to print
